Dronedarone (Multaq)

A non-iodinated amiodarone analog — fewer organ toxicities, but weaker and with hard limits after the ANDROMEDA and PALLAS trials.

Indication
Maintenance of sinus rhythm in paroxysmal or persistent AF in patients without recent decompensated heart failure or permanent AF
Typical dose
400 mg PO twice daily, taken with morning and evening meals

Why we use it

Dronedarone is the answer to the question “can we get amiodarone’s broad antiarrhythmic profile without the iodine-driven organ toxicity?” The answer is partly. It maintains sinus rhythm in paroxysmal and persistent AF with a cleaner long-term safety profile than amiodarone — at the cost of lower efficacy and a set of important contraindications.

Mechanism

Dronedarone is a benzofuran derivative of amiodarone with the iodine moiety removed and a methylsulfonyl group added. Like amiodarone it is multichannel — it blocks potassium (class III), sodium (class I), and calcium (class IV) currents and has antiadrenergic (class II) activity. Removing the iodine eliminates most thyroid toxicity and shortens the half-life to ~24 hours (versus weeks for amiodarone), so it does not accumulate the same way.

Dosing

  • 400 mg twice daily with meals. Food markedly increases absorption.
  • No loading dose; steady state in ~4–8 days.

The two contraindications that matter

  • Decompensated / advanced heart failure (NYHA IV or recent decompensation). The ANDROMEDA trial was stopped early for increased mortality in this group.
  • Permanent AF. The PALLAS trial showed increased cardiovascular death, stroke, and HF hospitalization when dronedarone was used in permanent AF. It must only be used to maintain sinus rhythm, and should be stopped if AF becomes permanent.

Monitoring

  • LFTs at baseline and periodically — rare cases of severe hepatocellular injury (including transplant) prompted a warning.
  • ECG — modest QT prolongation; assess at baseline.
  • Serum creatinine — expect a small rise from blocked tubular secretion; this is not true renal injury, but establish the new baseline.
  • Reassess rhythm status — confirm the patient has not progressed to permanent AF.

Interactions

  • Strong CYP3A4 inhibitors (ketoconazole, clarithromycin) markedly raise dronedarone levels — avoid.
  • Raises digoxin (reduce dose) and dabigatran levels (P-glycoprotein).
  • Additive QT effect with other QT-prolonging drugs; additive bradycardia with beta-blockers/CCBs.
  • Grapefruit juice increases levels.

Common pitfalls

  • Continuing dronedarone once a patient has drifted into permanent AF.
  • Using it as a rate-control agent — it is not.
  • Prescribing it in a patient with recent HF decompensation.
  • Forgetting the food requirement and getting subtherapeutic levels.
